- Please be ready to provide the name of your primary or referring physician, as well as the doctor
with whom you would like to schedule your appointment. Also, please have your insurance information and diagnosis available,
so that we can set up a consult with the physician.
Insurance Once we receive your
insurance information, we will verify your coverage. Depending on your coverage, we will provide the insurance company with
the necessary information for you to receive a referral or full authorization for your office visits, simulation, and/or treatment
appointments.
